• ALL STUDENTS, NEW AND RETURNING, ENROLLED IN KINDERGARTEN THROUGH GRADE 8 MUST SUBMIT THE F1 ANNUAL SCHOOL UPDATE FORM NO LATER THAN AUGUST 7^TH VIA EMAIL TO CGOEDDE@OLGCVA.ORG. This is a required annual emergency form per diocese and state regulations. No exceptions will be made.

• NEW to OLGC School families, regardless of grade, also need to submit a current (within 12 months) Commonwealth of Virginia MCH213 Form with medical provider signature and complete immunizations record. These are due by August 3^rd to Nurse Bliven jbliven@olgcva.org
  • ⬇️COMMONWEALTH OF VIRGINIA SCHOOL ENTRANCE HEALTH FORM (MCH213)
• ALL ENTERING KINDERGARTEN students, even if you are rising from OLGC Preschool, must submit a current (within 12 months) Commonwealth of Virginia MCH213 Form with medical provider signature and complete immunizations record. These are due by August 3^rd to Nurse Bliven jbliven@olgcva.org
• ALL 7^TH GRADE STUDENTS must have a FULL IMMUNIZATION RECORD sent to Nurse Bliven jbliven@olgcva.org showing 7^th grade required immunizations no later than August 3^rd.
